In Supported Living, each person receives support to live in their own home (which they lease, rent, or own) and acquire, maintain, or improve the skills necessary to live more independently. A person may receive services in the areas of self-care, health, wellness, meal preparation, personal resource management, communication, transportation, relationship development, and personal adjustment.

Each person is supported to participate in the community and use community resources. Transportation is included in Supported Living, except for transportation to and from competitive employment. Behavior Support Services are also included, if needed.

The type and amount of services a person needs are determined based on an assessment and the person centered planning process. Services are designed to support the person to achieve their goals, as stated in their individual support plan.

On-call staff are available 24 hours per day, if needed.
