Individual and Family Support Services

With Individual and Family Support Services, individuals with disabilities live in their own home or with their families. We strive to support people to make healthy choices and keep families together and involved in each others’ lives.

Our professional staff provide services necessary for individuals and their families to live healthy, productive lives. Individual and Family Support Services are based on a flexible, individualized support plan that is designed to meet the unique needs of each person. The number of hours of support may vary depending on each person’s needs.

Individuals are supported to improve their self sufficiency, socialization and adaptive skills.

Individual and Family Support Services are known as Home and Community Based Services and Home Based Support.
