Services: Home-based

Home-based Services are designed to provide support to persons with developmental disabilities on a very individualized basis. Support is primarily provided to persons who are living at home with their families, and may also be provided in other environments to enhance existing services. Home-based services are based on plans that are person centered and are designed to provide flexible and individualized supports that may include:

  • Providing "Hands-on" support in all aspects of daily living and personal care.
  • Providing support to participate in community integrative opportunities and activities.
  • Providing respite services.
  • Participating as a member of the person's Interdisciplinary Team.
  • Providing Behavioral consultation.
  • Providing goal plan development and implementation.
  • Providing training for family members or others identified by families as support providers for their family members.
  • Networking and support coordination with other Service Providers.
